DECLARATION OF CONSENT
Having taken cognizance of the contents of the information sheet received from the proprietor, IMAFORNI INT'L, in accordance with Art. 13 of Italian Decree Law no. 196/2003, and cognizant of the fact that I have full freedom of choice as regards the granting of my consent and that, should I refuse, IMAFORNI INT’L will not be able to respond to my requests, I hereby express my consent to the processing of my personal data, in accordance with Art. 23 of Italian Decree Law no. 196/2003, for the following purposes:
- the collection and storage of my personal data by IMAFORNI INT'L, for the supply of business information;
- the processing by IMAFORNI INT'L of my personal data, in accordance with the means and limitations indicated in the information sheet, in order to establish my business profile;
- the processing of my personal data for administrative and book-keeping purposes, including possible transmission via e-mail of business documents by IMAFORNI INT'L.
